For Jazmin, a Home Visitor Helped Jumpstart Her Career

New mom Jazmin realized her dream of going back to school with the help of home visiting program
Jazmin* knew her home visitor—matched through a home visiting program funded by the Connecticut Office of Early Childhood (OEC)—would help her navigate pregnancy, birth, and early parenting. What she didn’t foresee was that her home visitor would also help her achieve her career goals.
Jazmin’s home visitor was encouraged to go above and beyond thanks to an outcomes rate card with OEC. Since 2018, Social Finance has worked with OEC to develop six rate cards across its home visiting system that strengthen services by rewarding providers for achieving outcomes. The rate cards have focused service providers on priority metrics by tying a payment to the achievement of each metric.
When Jazmin shared with her home visitor that she dreamed of going back to school to become a nurse, the home visitor worked with her to develop a plan to make that happen. Even though home visiting is not a job training service, the home visitor helped Jazmin take classes, obtain a job as a Certified Nursing Assistant, and gain acceptance to nursing school—in part because the rate cards have included metrics related to supporting families’ education and employment goals.
In Jazmin’s case, the rate card meant that her own success was her home visitor’s success: the more she succeeded, the more the provider was paid by the state, and the greater the benefits Connecticut saw from its home visiting program.
